Interim Results of SED2010 Survey of Earned Doctorates (SED10)

for eligibility period from July 1, 2009 to June 30, 2010

Instructions:

Please review the table below. It lists the total number of SED10 graduates NORC has in our database for each commencement month, as well as the number of completed questionnaires received. (Keep in mind that it takes a short time for us to receive the materials you send.) Please read the following instructions and mail or fax the form back to us within the next THREE WEEKS. You may also phone or e-mail corrections to us.


1. Review the numbers of doctorate recipients for all commencement dates listed. If the number of graduates is correct, please place a checkmark next to the number. If the number is incorrect, write the correct number next to it.

2. If you have not already done so, please:

a. Send a FINAL LIST of graduates for all commencements within the eligibility period (July 1, 2009 to June 30, 2010. The final list helps us make sure our database is correct.

b. Send an annotated commencement program for all graduations within the eligibility period (July 1, 2009 to June 30, 2010)

3. Send in any COMPLETED QUESTIONNAIRES.
