OPM Form 2809 is used by annuitants
and former spouses to elect, cancel, suspend, or change health
benefits enrollment during periods other than open season. OPM Form
2809 has been revised to reflect the recent law that permits
children to retain family member health benefits coverage until age
26, bring the Public Burden Statement up-to-date, and make various
minor editorial changes (removed references to November 2005;
changed references to "out-of-wedlock" with "natural" children;
clarified several sentences.
