T ITLE OF INFORMATION COLLECTION: Heat Outreach Campaign Evaluation

Customer Satisfaction Survey


PURPOSE: In preparation for its 2012 heat outreach campaign, the Occupational Safety and Health Administration (OSHA) would like to conduct an evaluation of the 2011 campaign. The survey results will help OSHA to gauge the usefulness of the 2011 heat outreach campaign materials and to identify possible updates to the campaign for 2012.










DESCRIPTION OF RESPONDENTS: The respondents will be primarily local-level alliance and partnership members that OSHA compliance assistant specialists reached out and provided information to during the administration of OSHA’s 2011 heat outreach campaign. Each OSHA regional office has a list of these alliance and partnership members who form the potential pool of respondents. OSHA Alliance Program members include unions, trade or professional organizations, businesses, faith- and community-based organizations, and educational institutions. OSHA Partnership Program members include employers, unions, and trade councils and associations.

The respondents will be primarily local-level alliance and partnership members that OSHA compliance assistant specialists reached out and provided information to during the administration of OSHA’s 2011 heat outreach campaign. OSHA has already contacted its regional offices to determine the approximate number of respondents and will ask those regional offices for assistance in disseminating the survey. All potential respondents identified by the regional offices will be asked to complete the survey.

TITLE OF INFORMATION COLLECTION: Provide the name of the collection that is the subject of the request. (e.g. Comment card for soliciting feedback on xxxx)


PURPOSE: Provide a brief description of the purpose of this collection and how it will be used. If this is part of a larger study or effort, please include this in your explanation.


DESCRIPTION OF RESPONDENTS: Provide a brief description of the targeted group or groups for this collection of information. These groups must have experience with the program.
