Your child’s school is taking part in a study that is sponsored by the Centers for Disease Control and Prevention (CDC). The study is about school safety and violence and the physical features of schools.

SURVEY PROCEDURES

We would like your child to take part in a survey that asks a number of questions about safety and violence in schools. A select number of students who attend your child’s school are being asked to complete this paper-and-pencil survey. The survey will be done in class and takes about 40 minutes to complete. A member of the survey team will be there to help answer any questions.

POTENTIAL RISKS OR BENEFITS OF PARTICIPATION

Taking the survey will cause little or no risk to your child. The only potential risk is that some students might find certain questions to be sensitive. A school counselor or other school staff will be on hand to deal with any concerns your child may have. The survey has been approved by school officials. If you have questions about your child’s rights by taking part in this survey, you may contact Dr. Elizabeth Harris, Institutional Review Board Coordinator, toll-free at XXX-XXX-XXXX. A copy of the survey is at the school if you would like to see it.

CONFIDENTIALITY

The survey has been designed to protect your child’s privacy. Students will not put their names on the survey. Also, no school or student will ever be mentioned by name in a report of the results. This study may help children in the future by decreasing violence in schools. Your child will get no direct benefit from taking part in the survey. The only people who will know if your child is taking part in the survey are those who need to know, such as his/her counselor, teachers, and administrators. The survey results will be kept private to the extent allowed by law, and only study staff will be allowed to look at them. If you give permission, your child will be asked to complete this survey and will not be asked to take part in any other activities for this three-year study. Survey results will be kept for five years and will be stored in a locked filing cabinet or on a protected computer.

VOLUNTARY PARTICIPATION/WITHDRAWAL

We would like all selected students to take part in the survey but the survey is voluntary. No action will be taken against the school, you, or your child, if your child does not take part. Students can skip any question that they do not wish to answer. Also, students may stop taking the survey at any point without penalty.

CONSENT

SURVEY FACT SHEET

Q. Why is the CPTED Student Survey being done?

A. The purpose of the Crime Prevention Through Environmental Design (CPTED) Student Survey is to find out students’ views and experiences with crime and violence at school, including places where these experiences take place.

Q. What are the survey questions about?

A. Questions on the survey relate to student’s feelings about his/her school and how safe they feel in school.

Q. Can I see the questions my child will be asked?

A. Yes, a copy of the survey is at your child’s school.

Q. How was my child picked to be in the survey?

A. About 3,750 students from 50 middle schools in the Atlanta area were picked to take part in the survey. One or two classes from each grade 6 through 8 were picked at random to take part in each school. Your child’s class was one of those picked to participate.

Q. Will student’s names be used or matched to the surveys?

A. No. The survey has been designed to protect your child’s privacy. Specially trained field staff will administer the survey in each selected school. Students do not put their name on the survey. When students finish the survey, they place the survey in an envelope and seal it shut. The envelopes are then collected in a big box.

Q. Do students take the survey more than once to see how their behaviors change?

A. No. Students complete the survey only once. In addition, students cannot be followed because their names are not on the survey.

Q. How long does it take to complete the survey?

A. One class period (40 minutes) is needed to complete the survey.

Q. Does the survey include a physical test?

A. No. The survey does not include a physical test.
