Narrative of Changes
The purpose of the Narrative of Changes is to clearly indicate changes to a collection since the previous approval.

Current Expiration Date: January 31, 2012 (this has been extended from the previous date of
December 31, 2011
Collection Instruments: FEMA Form 086-0-32 (formerly FEMA Form 81-93)
The following are the changes to the collection:
FEMA received new comments following the 30 day notice of changes, a second set of changes have been made to meet the commenters’ needs. FEMA Form number has been changed from FEMA Form 81-93 and is now FEMA Form 086-0-32. This form has also been revised. Below are the sections that have been revised on the form:
Section I – Number 2 (Collateral) Simplified
Section II, B. 3 Simplified
Section II, C will be changed back to the previous (2008, FEMA form 81-93) form (this will simplify use).
Section II, D. New language - the last sentence will be removed
On the information pages (2-3):
#1. – add a sentence to the end of the paragraph “If other identifiers are available, such as Longitude/Latitude, please use any that will help describe the collateral location properly. Additional information may be attached.
#C. – two website URLs have been corrected due to change.
