This instrument is used by
Federally-regulated lending institutions, Federal Agency lenders,
the Federal National Mortgage Association, the Federal Home Loan
Mortgage Corporation, and the Government National Mortgage
Association. Federally-regulated lending institutions complete this
form when making, increasing, extending, renewing or purchasing
each loan for the purpose is of determining whether flood insurance
is required and available. FEMA is responsible for maintaining the
form and making it available but does not collect any information
itself.

The decrease in burden hours is
due to the conversion of this information collection to a common
forms collection to reflect that FEMA provides this instrument for
other Federal Agencies to use but does not collect any information
itself. The one burden hour is the minimum that ROCIS will accept.
No changes have been made to the information that is being
collected The decrease in burden cost is due to the conversion to a
common forms collection that is used by other agencies and not
collected by FEMA.
