RI 25-51 is used to collect
information from survivors of deceased annuitants who retired under
the Civil Service Retirement System. This form is used only for
survivors who are already identified in OPM records as payable when
the annuitants dies. The Department of the Treasury has directed
the Office of Personnel Management to inform applicants for
benefits that paper checks will no longer be issued to most payees.
Those who do not enroll in the Direct Deposit program so that their
payments will be deposited to their accounts in a financial
institution must contact the Department of the Treasury to obtain a
Direct Express debit card. RI 25-51 has been revised to contain
this information and the burden remains unchanged.
