Instructions for Tap Water TWQ Sample

  1. Remove the frozen ice packs from the tap water TWQ sample collection kit and place them in your freezer.

  2. Call FedEx (1-800-463-3399) to schedule a pickup or determine where to drop off the shipment. If FedEx cannot pick up the shipment or accept a drop off today, place the kit contents in a safe place and keep the ice packs in your freezer so you can collect the sample on another day.

  1. On the day of sample collection, remove contents of the TWQ sample collection kit from the shipping container (Photo 1).

  2. If possible, before collecting the sample, avoid handling or using the following: caffeine, tobacco, home cleaning agents, or personal care products, such as antibacterial soaps, hand sanitizers, or lotions.

  3. Place the sample ID labels on the Tap Water TWQ Questionnaire (question 4). Then write the date and mark the day of the week (questions 5 and 6).

To be done Monday through Thursday. The samples of your home tap water will be sent for laboratory analysis

  1. At your kitchen faucet, turn on the cold water and run it at a medium flow for 5 minutes.

  2. Put on the provided gloves. Gloves are to protect the sample from soap or oil on your hands.

  3. Open the aluminum foil with inner side up and place it on the counter next to your sink.

  4. Place both bottles included in the TWQ sample collection kit on the aluminum foil (Photo 2).

  1. If needed, reduce the water flow so the water will not splash when filling the bottle.

  2. Hold the first bottle upright and carefully unscrew the cap. Immediately begin filling bottle to the shoulder (Photo 3).

  • Do not rinse the bottle.

  • Do not fill the bottle completely full.

  • Do not pour out the preservative (white powder or liquid) inside the bottle.

  1. Immediately replace the cap after filling the bottle to the shoulder.

  2. Fill the second bottle as you filled the first.

  3. Label both bottles with the provided sample ID labels (Photo 4).

  4. Place the bottles in the foam inserts and then into the plastic bags. Label the bags with the remaining sample ID labels.

  1. Line the insulated shipper with the trash bag.

  2. Remove the ice packs from your freezer.

  3. Place five ice packs in a single layer on the bottom. Place the bottles on the ice packs. Place the rest of the ice packs around and top of the bottles. Fill the extra space with the provided packing material (Photo 5).

  4. Put the Styrofoam lid on and seal the insulated box.

  1. Write the date that the TWQ sample is picked up or dropped off on the prepaid, prefilled FedEx label, addressed to the laboratory, and attach it to the insulated shipper (Photo 6).

  • Ship the sample to the laboratory using FedEx.

  • This is prepaid. There will be no charge to you.

  1. Complete rest of the Tap Water TWQ Questionnaire and review it to make sure all questions were answered.

21. Place the Tap Water TWQ Questionnaire in the prepaid, preaddressed envelope. Seal the envelope and place in the U.S. mail.

  • Mail the questionnaire to the NCS Study Center Office using U.S. mail.
