Short Statement: Estimate public burden: number of respondents, frequency of responses, annual hour There are 4,116 PHAs and thus 4,116 respondents, each of which will be called upon to respond annually. It is estimated that the burden hour per response would be 20 minutes or less, based upon the ready availability of the relatively little amount of employee information requested to each respondent. The total annual hour burden for all respondents collectively is thus 1,372 burden hours. This collection does not use more than one form.
