ATTACHMENT #1 – Email Invitation

Good afternoon ARRA 5 Project Leads,

As discussed on several Disparities Subcommittee calls, one of the goals of the Evidence-Based Practice (EBP) Working Group is to establish the NCI Community Cancer Centers Program (NCCCP) Template for Community Outreach (Template) as an evidence-based model for implementing disparities-focused evidence-based programs. The Template was created for community cancer centers as a resource to assist sites in designing outreach programs to achieve the NCCCP program goals which are to: 1) enhance access to care; 2) improve quality of care; and 3) expand research opportunities particularly for racially and ethnically diverse and other underserved populations. The EBP Working Group is interested in understanding the process of and experiences with implementing the Template. The information obtained will be used to refine and improve the Template for future use.

The EBP Working Group would like to invite you to participate in a focus group with other NCCCP Disparities Leads to discuss the challenges and successes in implementing the Template at your site and contribute to improving the Template with your feedback. We will hold two focus groups on the following dates [INSERT DATES HERE] and have received Office of Management and Budget (OMB) approval to proceed with this information collection.

Please respond to Dr. Brenda Adjei at [email protected] to select the date you would like to participate. Please be advised that selecting a focus group date serves as your consent to participate in this focus group.

Sincerely yours,

Dr. Brenda Adjei
