Forms SS-8 & SS-8 (PR), Determination of Workers Status for Purposes of

Federal Employment Taxes and Income Tax Withholding

Description of Major Changes for Form SS-8 (Rev. August 2011)

The following major changes have been made to the form, per the SS-8 Unit and General Counsel emails of 3/8/2011:

• Lines for Case Number and Earliest Receipt Date were added under the OMB number.

• In the firm and worker name and address section, boxes were added for:

• Firm and Worker email address

• Firm and Worker fax number

• Firm’s fax number and Firm’s website

• Worker’s alternate telephone number

• In Part I, a new line was inserted after line 6, and lines 7-10 were renumbered.

• The headings for Parts II and III were revised.

• In Part III, line 2, 5, 8, and 9 were revised and line 10 was added.

• In Part IV, lines 1, 3, 6, 8, 9, and 10 were revised.

• In Part V, lines 8 and 9 were revised.

• In the General Instructions, minor editorial changes were made to the sections for the Form SS-8 Determination Process, Completing Form SS-8, Fee, Where To File, Instructions for Workers, and the Privacy Act and Paperwork Reduction Act Notice.

• A Spanish version of Form SS-8 (PR)/instructions is being made available for use in Puerto Rico.
