The three FCC forms (FCC Forms 479, 486, and 500) serve the functions of the Universal Service Schools and Libraries Support Mechanism (47 U.S.C. Section 254, et seq.). They are used at the point where services provided pursuant to the Program are implemented; or about to be implemented and are a necessary prerequisite to the distribution of payments under the Program. The forms ensure that congressional mandates regarding access to the Internet by minors are met.
The Federal Communication Commission is seeking a non-substantive change request for information collection 3060-0853. The Commission received preapproval from the Office of Management and Budget (OMB) for the information collection requirements that were contained in FCC 09-96 on March 25, 2010. The Commission adopted the requirements as proposed in FCC 11-125 on August 10, 2011. The Commission is now submitting this change request to activate the burden hours of 7,500 hours into OMB's inventory and also to provide OMB with updated copies of the instructions for FCC Forms 479 and 486. FCC Form 500 was not impacted by this submission to OMB.

The Commission has program changes/increases of 30,000 to the number of respondents, 30,000 to the annual number of responses and 7,500 hours to the annual burden hours which are due to the adoption of the information collection requirements contained in FCC 11-125.
