morning!
My
name is _______________. I am here on behalf of the Centers for
Disease Control and Prevention (CDC) and the Red Cross doing an
assessment of water, sanitation and hygiene practices here in
___________________.
The
purpose of this evaluation is to do a follow-up on the state of water
quality, sanitation and hygiene practices in this community after the
activities completed by the Red Cross after Hurricane Mitch in 1998.
We
did other evaluations in 2002, 2006 and 2009. We are doing this
evaluation at the 10-year mark.
I’d
like to talk to the person that prepares the food and has the
responsibility for the house and collects water for the home. During
this interview, we will ask about water use in the home and take a
water sample to test for bacteria. We will also ask about the use of
latrines, and ask about any health education that you have received.
We ask your cooperation and we would like to assure you that:
Your participation is anonymous (your name will not be on the questionnaire and the
results will be presented in general terms, not by person)
Your participation in this study is completely voluntary. You do not have to answer any question that you do not want to answer.
We appreciate your participation. Your answers will help to improve programs and services in your community. We believe that with your participation in this assessment is important. However, if you do not want to participate or if you have any problems with participating, that is ok. Do you have any questions?

Mi nombre es _______________. Estoy aquí por parte de los Centros para el Control y Prevención de Enfermedades (CDC) y la Cruz Roja para realizar una evaluación de agua, saneamiento y prácticas higiénicas aquí en ___________________.
El propósito de este estudio es de hacer la última evaluación sobre la calidad del agua, el saneamiento y prácticas de higiene en esta comunidad después de las actividades realizadas por la Cruz Roja a causa del Huracán Mitch en 1998. Hicimos otras evaluaciones en el 2002, 2006 y 2009. Esta evaluación será la última para completar este estudio de diez años.
Me gustaría platicar con la persona que se encarga de prepara la comida, cuidar la casa y que recolecta el agua para la casa. Durante esta entrevista, le preguntare sobre el uso del agua en el hogar y hare un análisis sencillo para determinar la presencia de bacteria en el agua, le hare preguntas sobre el uso de los servicios sanitarios y de cualquier educación sobre la salud que le hayan dado.
