The Centers for Disease Control and Prevention (CDC) Community Interventions for Infection Control Unit (CI-ICU), with the assistance of Oak Ridge Institute for Science and Education (ORISE), is conducting a Web-based survey to assess the guidance, communication, training, and monitoring needs of state, tribal, local and territorial health departments (STLTs) around nonpharmaceutical interventions (NPI) and pandemic influenza. It will also help CDC develop NPI materials and training for STLTs. No personal identifying information will be requested.


Your feedback is important, as it will help CDC support your public health efforts around implementing, communicating, and monitoring NPIs before, during, and after an influenza pandemic. The survey contains 21 items and should take about 20-25 minutes to complete.


Due to software requirements, the survey must be completed and submitted online in one session. Please submit your survey online by [actual date TBD pending OMB approval].
