Attachment M – Advance email notification of data collection for non-CODES participants

Dear Injury Prevention Partner,

Motor vehicle crashes are the leading cause of death in the U.S. accounting for over 30,000 deaths a year (NHTSA). The National Highway Traffic Administration’s (NHTSA) Fatality Analysis Review System (FARS) provides an excellent source of data on motor vehicle crash fatalities at both the state and national level. However, serious injuries are not tracked in a similar database. The Centers for Disease Control and Prevention (CDC) has priorities of strengthening surveillance and epidemiology and providing state and local support; providing state and national level data on motor vehicle crash injury fits these priorities. To meet this need, the CDC and NHTSA through the Crash Outcome Data Evaluation System (CODES) program are working collaboratively to obtain information on how to build a system infrastructure with the necessary attributes identified.

It is for this reason that your public health agency is being contacted. In two weeks we will be sending a survey to you by email which we would like for you or a representative to complete. The information will be used to document those attributes that would need to be available and in place to make a state or community successfully in conducting such surveillance and analysis.

We ask that your agency take the time to complete the attached survey and/or to pass this information along to your State’s CODES contact person.
