Hello, my name is _____________ from _____________. Today I am calling on behalf of ATSDR to ask questions about documents ATSDR produces. This call does not involve sales of any kind – we are only interested in your opinions.

As you know ATSDR produces public health assessments and health consultations in order to communicate the results for evaluating the public health implications of exposures to environmental contamination. ATSDR recently completed a document for the [Navy Mills Health Consultation/BoRit Asbestos Health Consultation/Black Butte Mine Public Health Assessment/Oak Ridge Y-12 Mercury Release Public Health Assessment/Midlothian Air Quality Health Consultation] site. We are contacting state environmental health employees and officials at 5 sites across the country to get feedback on how we can improve these documents and ascertain how you used the document. In order to improve the documents ATSDR produces we are running focus groups with individuals who have read and/or used the document and we would like to invite you to join us. The focus group is completely voluntary and in no way will your state be identified with the answers you provide. The information you provide will enhance the usefulness of documents to our stakeholders.
