Attachment D – Introductory email

Dear <xxxx>

Below please find the link to an online assessment tool which will capture information on regulations in your jurisdiction regarding the sale, display, and exhibition of certain animals. You have been invited to participate in this assessment due to your role as a public health veterinarian or official who handles zoonotic issues for your jurisdiction.

Public health continues to face a number of human outbreaks related to certain animals and it is important to catalogue the laws in each jurisdiction. During 2012 greater than 1000 persons were sickened due to contact with birds sold through mail order hatcheries, turtles with a shell length less than 4 inches, hedgehogs, and frozen feeder rodents. Your individual responses will be anonymous; data will only be presented publicly in aggregate form. This assessment has been reviewed and approved under OMB No. 0920-0879.

The link is individualized to you, allowing you to save and restart the assessment at any time; for that reason the link cannot be forwarded to another individual. In addition, the personalized link below allows you to start and save the assessment as needed so that it does not have to be completed at one time. We estimate it will take approximately 17 minutes to complete the assessment. If you feel that you are not the most appropriate person from your jurisdiction to respond, please contact Jennifer Wright, DVM, MPH at 404-639-4749 or [email protected] and we will be happy to send a new link to the appropriate person.

Thank you,

Jennifer Gordon Wright, DVM, MPH, DACVPM
