Dear Colleague,

As you may know, CDC’s Office for State, Tribal, Local and Territorial Support (OSTLTS) was formed in 2010 to oversee a breadth of crosscutting programs that increase CDC’s support and technical assistance to state, tribal, local, and territorial (STLT) health agencies. In order to achieve our work, OSTLTS developed a host of communication products and tools to encourage communication with the STLT community.

In order to further improve and strengthen our communication products, we would like to invite state, tribal, local and territorial public health professionals who subscribe to our products to participate in a 15-minute assessment. Your feedback is invaluable and will be used to refine our communication products so as to better meet your needs.
