Dear Dr. X,
I am an epidemiologist with the Enteric Diseases Epidemiology Branch at CDC. Last week I sent you an email to let you know about some information that we are conducting in order to better understand the national picture of the epidemiology and laboratory surveillance capacity of State Health Departments for Campylobacter and Shiga toxin-producing E. coli (STEC). Our records indicate that we have not yet received a response from your state and your participation is very valuable to us. The online questionnaire contains 13 questions takes about 10 minutes. In addition to updating our knowledge of how Campylobacter and STEC surveillance is being conducted at the state level, we will summarize the responses from all contacted health departments and send that out in a final report.
