Dear Dr. X,
I am Team lead of the National Enteric Reference Laboratory at the Enteric Diseases Laboratory Branch at CDC. A couple of weeks ago you received an email letting you know about some information that we are conducting in order to better understand the national picture of laboratory surveillance capacity at the state public health laboratories for Campylobacter and Shiga toxin-producing E. coli (STEC). Our records indicate that we have not yet received a response from your state and your participation is very valuable to us. The online questionnaire contains 24 questions takes about 15-20 minutes. In addition to updating our knowledge of Campylobacter and STEC testing capacity and procedures at the state level, we will summarize the responses from all public health laboratories and send that out in a final report.
