Short Statement: There was a program change, increasing the number of forms from two to four, to accurately reflect the number of forms being used to collect this information. There was an increase to the annual cost burden of $10 for respondents which reflects the amount, for postage and envelopes, to submit the forms which was not accounted for previously. Also, adjustments were made to the hour burden to reflect the accurate number of respondents, reducing the number of respondents from 21 to 18.
