The Facility Compliance Calculation Report must be submitted annually. In the case of Truck Loading
Terminals (the point where D&T ends), this report must be submitted quarterly. See section headings for
reporting notes and reference the regulations for detailed descriptions and definitions of the formulas and
their parameters.
Please do not leave any reporting fields in this report blank. When reporting, enter “NA” for fields that are
not applicable unless otherwise specified.
Reports using this form must be submitted quarterly in the case of Truck Loading Terminals with due
dates of November 30th, February 28th, May 31st, and August 31st. Annual reports using this form must
be submitted by August 31st.
