<<DATE>> Dear
Airman,
Office
of Aerospace Medicine 800 Independence Ave., S.W. Washington,
D.C. 20591
As
you know, medical certification of airmen is an important safety
function for the Federal Aviation Administration. I would like to
invite you take part in a survey about your satisfaction with
aerospace medical certification services as delivered by Aviation
Medical Examiners, FAA Regional Flight Surgeons, and the FAA
Aerospace Medical Certification Division in Oklahoma City. Knowing
what airmen think about their experiences with the medical
certification process is vital to our improvement efforts.
Because
there are more than 600,000 active airmen, this survey invitation is
being sent to a random sample of airmen from varying category and
class ratings who have recently sought medical certification from an
Aviation Medical Examiner. You have been selected to serve as a
representative of your category and class rating, so it is important
that you complete and return the survey (<<<OMB #>>>).
You
may complete the survey online via the Internet at: <<<<WEB

If
you prefer a paper copy of the survey, or if you have difficulty
accessing the website, please contact Suzanne Thomas by e-mail at
[email protected]
or by phone (405) 954-1594. Please
choose only one method to complete the survey.
Your
participation is voluntary
and your responses will be anonymous.
All data will be reported in the aggregate.
Your opinion is very
important to us but will not be counted unless you respond to this
survey. So, please take a few minutes to complete the survey now.
Thank you for helping us to
serve you better. With your help, we will continue to provide the
safest aerospace system in the world.
Sincerely,
Frederick
E. Tilton, M.D.
