Abstract: In accordance with DoD 7000.14-R, Volume 5, an applicant must identify himself/herself to ensure the DoD Stored Value Card (SVC) is issued to the correct person. The DD Form 2887 is used to maintain a record of individuals enrolled in the SVC program and to authorize the transfer of funds from the individual's bank account to their SVC.
