Letter of Introduction to Potential Eligible Trailer Occupants

Dear [Registrant],

The Agency for Toxic Substances and Disease Registry (ATSDR) is forming a Katrina Pilot Registry to find out about the health of those who lived in FEMA trailers after Hurricanes Katrina and Rita. What we learn from the pilot registry will be used to decide what sort of research we should do. We will also try to find out:

• If we can contact and enroll people who lived in FEMA trailers in a registry

• How much it will cost and what will need to be done to make a larger registry

• Whether people will answer the questions we want to ask about their overall

health and breathing problems

We are inviting people who lived in a FEMA trailer to take part in the pilot registry. FEMA is funding this survey.

The pilot registry will ask questions about:

• Where you lived before and after the hurricanes

• Your age, sex, and racial background

• Your health

This pilot registry will help us learn more about ways to improve health and prevent disease in the future. We will be calling you in the next few weeks to interview you for the pilot registry. The interview should take about 25 minutes of your time to complete.

You do not have to take part in the pilot registry. There are no penalties if you decide not to take part. All answers you give will be kept private as much as the law allows. We will not use your name in any reports from this pilot registry.
