INSTRUCTIONS FOR COMPLETING GROWERS LISTS
1. INTERVIEWER'S ID (OR NAME). IF MORE THAN ONE INTERVIEWER IS ASSIGNED TO THE COUNTY, ENTER THEIR ID NUMBERS (OR NAMES).
2. ENTER NUMBER OF ATTEMPTS, DATE AND TIME OF EACH ATTEMPT (MINIMUM THREE ATTEMPTS). INCLUDE METHOD USED FOR ATTEMPT: IN PERSON OR BY PHONE.
3. If comments reads "Address Located out of county, ATTEMPT TO CONCACT!!!" you must attempt to contact and document method(s) used to confirm location(s) of farm.
4. WRITE NAMES OF PERSONS CONTACTED (EACH TIME) AND THEIR CORRESPONDING TITLES (i.e., secretary, owner, foreman, etc.).
5. IF REFERRED TO A CONTRACTOR, WRITE CONTRACTOR'S NAME, ADDRESS, AND TELEPHONE NUMBER.
6. DOCUMENT NUMBER OF EMPLOYEES AT TIME OF CONTACT (SEASON).
7. DOCUMENT NUMBER OF EMPLOYEES CONTACTED WHO REFUSED TO PARTICIPATE.
8. CORRECTIONS: ENTER NEW NAME, ADDRESS, AND DIRECTIONS ON HOW TO GET TO FARM/NURSERY.
9. COMMENTS BY CONTACTED PERSON, ETC.
10. NARRATIVE: COMMENTS BY CONTACTED PERSON; SEASONALITY; IF REFUSAL, WRITE REASONS; ANY OTHER COMMENT THAT MAY HELP UPDATE LISTS.
11. FOR NARRATIVE, IF NOT ENOUGH SPACE, WRITE ON BACK OF PAGE.
12. "Grower Return Rule": When a grower agrees to participate after you have completed the allocated amount, return to this grower and conduct at least one interview.
13. "Last Grower Rule": On the last grower you conduct surveys to complete the allocation, conduct the maximum number of interviews per grower. (i.e.., County allocation is 12 surveys, if 10
surveys are completed, do 5 surveys at the last grower for a total of 15 surveys).

***Remember: Only workers 14 years or older can be surveyed. H2-A workers cannot be surveyed.
