PART I
This survey contains questions regarding your experiences before, during, and after military
deployment. No one has had exactly the same experiences that you have had, so your input is very
important. There are no right or wrong answers.
Be sure to answer every statement, and press firmly on the page when circling your responses. If
you want to change your response, please place an “X” over your original answer, and then circle
the more appropriate response.
