Women’s Health Study
The purpose of this study is to improve the way information about rape and sexual assault is collected. Westat, a social science research organization in Rockville, Maryland, is working with the U.S. Bureau of Justice Statistics (BJS) to develop and test these survey methods. As part of this project, Westat is recruiting female survivors of rape or sexual assault to help in testing the questions being considered for a new survey. Beginning in February 2013, Westat would like to speak with women who are: Age 18 and older, English speaking, and a previous victim of rape or sexual assault. These interviews will be completed in person, at the DC Rape Crisis Center, and will last about 60 minutes. Respondents will receive $40 for the interview as a token of appreciation for their time.
Please contact Indira at [email protected] or 202-232-0789 if interested in participating.

ACASI screener and cognitive interview protocol
Introduction
Thank you for agreeing to talk with me today. Let me tell you a little about Westat and what we’re going to do today.
Westat is a research company that conducts research on many different topics, under contract with many different organizations. This particular project is for the Department of Justice, specifically, the Bureau of Justice Statistics. Westat has been contracted to develop and a survey about unwanted sexual experiences. We will be interviewing thousands of women nationwide for this survey.
At this stage we are still developing the questionnaire and trying to figure out how to make it as easy as possible for people to understand and answer the questions. . So the way this will work is I will ask the questions as if I were an interviewer sitting across from you in your living room. And you’ll answer them just like we were doing a normal survey. As best you can, try to think out loud as you answer the questions. At various times in the interview we’ll stop and talk about the questions and how you answered them. For example, I may ask you to explain your answer in more detail or I might ask questions such as:
How did you come up with your answer; or
How did you remember that; or
What did it sound like that question was asking to you?
We are very interested in your feedback on the questions. At any time you should feel free to let me know if—:
there are words you don’t understand;
the question doesn’t make sense to you;
you could interpret it more than one way; or
the answer you are looking for is not provided.
These follow up questions may seem strange, but bear with me. They help me understand how well the questions are working. In particular, we are trying to find out how people like you go about answering these questions. So, basically, the more you can tell us, the more we will understand about the questions.
Before we get started, I want to let you know that everything we cover in the interview will be kept private - only the few people actually working on the project will have access to the information you share with us. If there are any questions you don’t want to answer that is perfectly fine - We’re really more interested in how you interpret the questions.
If it’s OK with you, I would like to record our conversation, just so that I can review it later when we’re analyzing all the interview data. If it is OK, I’d like to get your agreement on the recording.
Do you have any questions before we begin?
Interview begins here.

Event History Calendar
COGNITIVE INTERVIEWER
INSTRUCTION: The
two main goals of this section are to understand (1) how long it
takes to administer the event history calendar, and (2) how useful
the questions are for placing events on the calendar. You don’t
need to spend more than about five minutes on this section.
Introduction: Before we move on to questions about health and injuries, let’s spend a few minutes talking about what you did and things that happened to you over the last year. It might be difficult to remember things that happened as long as a year ago. Sometimes people find it helpful to think about a calendar to remember things. Let’s note some dates on the calendar. Looking back a year ago, there was Thanksgiving in November of 2012, New Year’s Day in winter, St. Patrick’s Day in the spring, Memorial Day in May, the Fourth of July in summer, and Halloween in October. Now let’s put some things that are specific to you on the calendar.
Interviewer instruction regarding boundary events.
PROMPT 1: What are some of the things you did or things that happened to you this past year, that is, since <REFERENCE MONTH> 1st of 2012?
RECORD RESPONDENT’S EVENTS ON CALENDAR. TRY TO GET AT LEAST ONE EVENT AT THE BOUNDARY. COLLECT MORE EVENTS IF R IS ABLE TO NAME MORE.
READ PROMPT 2 IF RESPONDENT HAS NOT PRODUCED A BOUNDARY EVENT:
PROMPT 2: Are there dates for things you did or things that happened since last <REFERENCE MONTH> 1st that we can note on the calendar? It doesn’t have to be anything unusual or important, just anything that you remember from the past 12 months.
RECORD RESPONDENT’S EVENTS ON CALENDAR. ASK A GENERAL PROMPT SUCH AS “ANYTHING ELSE?” TO ELICIT MORE EVENTS.
READ PROMPTS 3a-d IF RESPONDENT HAS NOT PRODUCED A BOUNDARY EVENT:
IF RESPONDENT HAS NOT MENTIONED A FAMILY MILESTONE, READ:
PROMPT 3a. Were there any family events, such as a birth or birthday celebration, a wedding, or a death in the family?
IF RESPONDENT HAS NOT MENTIONED A VACATION OR FAMILY EVENT, READ:
PROMPT 3b. Did you or anyone in your family go on vacation or to a special event?
IF RESPONDENT HAS NOT MENTIONED A WORK-RELATED EVENT, READ:
PROMPT 3c. Did you change jobs, or get a promotion?
IF RESPONDENT HAS NOT MENTIONED CHANGING RESIDENCES, READ:
PROMPT 3d. Did you move to a different house or apartment?
MARK EVENTS ON THE CALENDAR. READ PROMPT 4 IF RESPONDENT HAS NOT PRODUCED A BOUNDARY EVENT
PROMPT 4a? Where are you living now? [NOTE ADDRESS ON CALENDAR] How long have you lived there? Did you move in the last 12 months, since <REFERENCE MONTH> 1st of 2012?
PROMPT 4b? Do you work or attend school? Where are you working/attending school now? How long have you worked there/attended that school? [RECORD JOBS AND SCHOOLS ON CALENDAR.] Did you work at any other jobs or attend another school in the last 12 months, since <REFERENCE MONTH> 1st of 2012?
READ PROMPT 5 IF NO BOUNDARY EVENT OBTAINED FROM <REFERENCE MONTH> of 2012.
PROMPT 5: We are interested in getting events from the entire year, beginning in <REFERENCE MONTH> of 2012. Can you think of anything from your life to put on the calendar for <REFERENCE MONTH> of 2012?
COGNITIVE INTERVIEW PROBES
FOR RESPONDENT: Was
there anything you didn’t understand about the calendar? Which
questions gave you the most help coming up with events to place on
the calendar? Is
there anything else that would have helped you place events on the
calendar?
SV Screener
COGNITIVE INTERVIEWER
INSTRUCTION: The
goals for this section are to (1) understand the general
comprehension of the screener items, (2) evaluate alternative
question wordings, and (3) understand if multiple reports represent
distinct incidents or are part of the same incident.
Complete
SV1-8 and then come back to ask follow-up probes. Some questions
have specific probes, but you can probe other questions as needed.
Once finished with these questions, proceed to SV9-13.
You
may probe concurrently on questions where you feel that there is a
problem. In general use expansive probes that encourage the
respondent to explain their answer in more detail. For example, if
the respondent reports an incident you may follow-up with probes
like—How did you come up with your answer? Tell me what you
are thinking about. Tell me more about your answer. If the
respondent does not have an incident to report, you may follow-up
with a probe like “What is that question asking about?”
SV INTRO:
People may experience unwanted and uninvited sexual situations by strangers or people they know well, such as a romantic partner, friend, acquaintance, student, teacher, coworker, supervisor, landlord, or family member. Sometimes these situations result in injuries or other types of harm. Your answers to the next set of questions will help us learn how often these things happen.
Some of the language we use is explicit, but it is important that we ask the questions this way so that you are clear about what we mean. The questions we ask are detailed and some people may find them uncomfortable. The information you are providing will be kept private. You can skip questions you don’t want to answer and you can stop at any time.

Westat has been contracted to conduct a survey about unwanted sexual experiences. For this survey, we will be interviewing thousands of people nationwide.
At this stage we are still developing the questionnaire and trying to figure out how to make it as easy as possible for people to understand and answer correctly. So the way this will work is I will ask the questions as if I were an interviewer talking with you over the phone. And you’ll answer them just like we were doing a normal survey. As best you can, try to think out loud as you answer the questions. At various times in the interview we’ll stop and talk about the questions and how you answered them. For example I may ask you to explain your answer in more detail or I might ask questions such as—
How did you come up with your answer;
How did you remember that; or
What did it sound like that question was asking to you?
These questions are basically rough drafts. At any time you should feel free to let me know if—
There are words you don’t understand;
The question doesn’t make sense to you;
You could interpret it more than one way; or
The answer you are looking for is not provided.
CONSENT FORM: Here is a form I must ask you to look over and sign – it basically covers the points I’ve just gone over with you and indicates you have agreed to take part [ONE COPY FOR RESPONDENT, ONE COPY FOR US]
Do you have any questions before we begin?

SV Screener
COGNITIVE INTERVIEWER
INSTRUCTION: The
goals for this section are to (1) understand the general
comprehension of the screener items (2) evaluate alternative
question wordings and (3) understand if multiple reports represent
distinct incidents or are part of the same incident.
For example, if
the respondent reports an incident you may follow-up with probes
like— How did you come up with your answer? Tell me what you
are thinking about. Tell me more about your answer. If the
respondent does not have an incident to report, you may follow-up
with a probe like “what is that question asking about?”
SV INTRO:
People may experience unwanted and uninvited sexual situations by strangers or people they know, such as a romantic partner, friend, teacher, coworker, supervisor, or family member. Sometimes these situations result in injuries or other types of harm. Your answers to the next set of questions will help us learn how often these things happen. Most questions only require a yes or no response.
Some of the language in the survey questions is explicit, but it is important that we ask the questions this way so that you are clear about what we mean. The questions we ask are detailed and some people may find them uncomfortable. The information you provide will be kept private. You can skip questions you don’t want to answer and you can stop at any time.
I will ask about different types of unwanted sexual situations that may or may not have resulted in an injury or other type of harm. Most of the questions just require you to say ‘yes’ or ‘no’ to answer, and you may be asked for a month or year as a response. Are you in a private place where you feel comfortable answering?
SV1. Has a man or boy ever made you have sex by using force or threat of harm? By sex we mean putting a penis in your vagina.

Detailed incident form and cognitive interview protocol
COGNITIVE INTERVIEWER
INSTRUCTION: The
general approach for the detailed incident form is to ask the
questions within each section and then go back and probe on that
section. We are primarily testing (1) comprehension of the response
categories and (2) flow of the detailed incident form. Below are the
points of interest in each section.

the Incident
COGNITIVE INTERVIEWER
INSTRUCTION: (If
you have not already covered the details of the incident being asked
about with previous probing on the screener) Before we move forward,
can you describe for me what happened?
(You said that during the past 12 months – (Refer to appropriate screen question for description of crime.) and that it happened (number) time(s). The next questions are about the details about (this incident/the most recent time this happened) . We may ask you something that you already told us. We want to make sure that we have the details correct.

Cognitive interview protocol for the second interview
Introduction
Thank you for agreeing to come back and talk with us today. This second interview will help us improve our interviewing procedures and how we ask questions.
Last time you may recall that we often stopped to talk about the questions as you went through the questionnaire. Today we will have you answer some of the same questions that you did before. However, we will not be talking about your answers until after you are finished going all of the way through the questionnaire.
Then at the end of the questionnaire we’ll talk about how everything went this time.
[PROCEED TO INTERVIEW]
General procedures
[BRING RESPONDENTS ANSWERS TO THE SCREENER AND DETAILED INCIDENT FORM FROM THE FIRST INTERVIEW TO THIS INTERVIEW. FOLLOW THE RESPONDENT THROUGH THE INTERVIEW AND MAKE NOTE OF ANY DISCREPANCIES BETWEEN THE FIRST AND SECOND INTERFVIEW.]
[BEGIN WITH SVINTRO1 AND SVINTRO2 AT THE BEGINNING OF THE SEXUAL VIOLENCE SCREENER. ALLOW THE RESPONDENT TO GO ALL THE WAY THROUGH THE SCREENER (SECTION SV). THEN SELECT THE FIRST INCIDENT REPORTED (IF ANY) AND ALLOW THEM TO GO ALL THE WAY THROUGH THE DETAILED INCIDENT FORM. IF THERE IS TIME, ALLOW THEM TO COMPLETE THE DETAILED INCIDENT FORM FOR ALL INCIDENTS REPORTED ON THE SCREENER.]
[IN THE EVENT THAT YOU NOTICE ANY DISCREPANCIES, YOU MAY ASK SOMETHING LIKE THE FOLLOWING] We asked you [INSERT QUESTION]. One time we recorded YES and one time we recorded NO. What is the best answer?
[FOLLOW-UP AS NEEDED TO UNDERSTAND THE REASON FOR THE DISCREPANCIES.]
[IF THE RESPONDENT DOES NOT REPORT ANY INCIDENTS ON THE SCREENER AND THEY DIDN’T REPORT ANY INCIDENTS IN THE FIRST INTERVIEW, THEN ADMINISTER THE VIGNETTES AND DEBRIEFING QUESTIONS AND PROBE AS NECESSARY ON EACH.]
