The agency adjustment in the annual number of responses is due to a decrease in respondents each year for the past 3 years. The time per respondent remains 30 minutes, however the reduction in the number of respondents has caused the annual burden hours to decrease to 513. The cost per respondent has increased to $.45 due to postal rate increases, however the annual cost has decreased to $461 due to the reduction in the number of respondents.
