Attachment B – Collection of Documentation
Below is a list of documents we would like to collect. We are interested in learning more about quality improvement and system redesign efforts at your institution and realize that only some of these documents may contain relevant information. The collection of these documents is completely voluntary and we only ask you collect those documents that are readily available. The list below is not exhaustive, please collect any documents you think might help us understand the recent and ongoing system redesign and quality improvement efforts at your institution.
Annual Report – 2006 – 2011
Recent performance dashboards – including quality dashboard if separate
Final, Interim or Project Reports on recent quality improvement projects or system redesign projects
Recent electronic and/or paper Hospital newsletters
Any relevant communications to staff or patients regarding quality improvement initiatives or system redesign efforts
