				To Managers of Mills, Elevators, Warehouses, and Other Storage Facilities: Please fill out and mail or fax this report by December 9, 2011. Under Title 7 of the U.S. Code and CIPSEA (Public Law 107-347), facts about your operation are kept confidential and used only for statistical purposes in combination with similar reports from other producers. Response is voluntary.

• If your firm operates plants in more than one State, separate reports should be prepared for each State.

• Report total capacity and stocks in storage facilities operated by your firm.

• If your firm operates more than one Mill, Elevator, or Warehouse in Iowa,

■report total data for all storage facilities if individual storage facilities do not report.

■report only your storage facilities data if individual storage facilities do report.

• Include both bulk and sacked capacity in the rated storage capacity.

• Include all grain stored in your facilities and/or piles (temporarily stored) regardless of ownership for farmers, elevators, millers, processors, farmer owned reserve, under loan, and the Government Commodity Credit Corporation. Include grain to be used for seed.

• Exclude grain owned and/or stored at another licensed grain elevator.

• If no stocks are on hand, please check and return this report.
