B. Purpose


The ETA 9128u report provides quarterly information on Re-employment Services and Re-employment and Eligibility Assessments (RES/REA) for EUC08 claimants. Public Law 112-96 requires states with an EUC08 agreement to provide RES/REA services to most claimants starting Tier 1 after March 23, 2012, and to most claimants starting Tier 2 after March 23 who did not previously receive RES/REA services. Required services include provision of labor market and career information, skills assessment, orientation to One-stop Career Center services, and a review of EUC08 eligibility relating to job search. Additional re-employment services may also be provided. The data on this report allows for monitoring of RES/REA activities and will be used to determine the amount of funding to be provided.


C. Due Date and Transmittal


The electronically submitted report is due in the ETA National Office on the first day of the second month following the quarter of reference (e.g. August 1 for the April-June quarter). The first report is due August 1, 2012.


D. General Reporting Instructions


Data on this report deals with individuals in the EUC08 program only. State Workforce Agencies (SWAs) or other agencies administering RES/REA activities should ensure that systems are in place to collect information on service receipt.


Data are reported for the quarter in which the activity occurred. The activity being reported could occur one or more quarters after the quarter in which the individual was first scheduled for RES/REA services.


If there is any reportable RES/REA activity in March 2012, report it on the April-June 2012 report.

E. Item by Item Instructions


1. Number of Claimants First Scheduled for RES/REA. Enter the number of claimants who were scheduled for RESREA for the first time during a period of EUC receipt based on a particular benefit year. If the claimant failed to report for RES/REA and was rescheduled, the rescheduled RES/REA should not be counted in this item. If a claimant is again scheduled for RES/REA based on a subsequent benefit year, the claimant should be counted again.


2. Number of Claimants Provided All Required RES/REA Services. This item will be used to determine the amount of funding to be provided to the state. Enter the number of claimants reported in item 2 who were provided all services required under Public Law (P.L.) 112-96. If the services were provided at different times, report the claimant in the quarter in which the final service was provided. Include claimants who were excused from participation in one or more, but not all, of the required services. If one or more services require follow-up action by the claimant, count that claimant as having been provided all services whether or not the claimant took that action.


3. Number of Claimants Receiving Additional Services. Enter the number of claimants reported in item 2 who received any additional services that are permitted, but not required, under P.L. 112-96 during the report quarter.


4. Number of Claimants in Item 2 Disqualified. Enter the number of claimants reported in item 2 who received required RESREA services and were disqualified or were ineligible for benefits for a week or more. The disqualification or ineligibility must be the direct result of the eligibility review.


5. Number of Claimants in Item 4 Disqualified for Work Search Issues. Enter the number of claimants reported in item 4 who received required RES/REA services and were disqualified or were ineligible for benefits for a week or more based on a work search issue. The disqualification or ineligibility must be the direct result of the eligibility review. Claimants reported in item 5 should also be reported in item 4.


6. Number of Claimants Who Failed to Report for the Initial RES/REA Session. Enter the number of claimants who were scheduled for an initial RES/REA session who failed to report. It includes claimants who failed to report and who were subsequently rescheduled for RES/REA at a different time. Claimants counted in this item should also be counted in items 7, 8, or 9.


7. Number in Item 6 Who Were Rescheduled Without Disqualification Enter the number of claimants reported in item 6 who were rescheduled and were not disqualified or ineligible for benefits for a week or more.


8. Number in Item 6 Who Were Disqualified. Enter the number of claimants reported in item 6 who were disqualified or were ineligible for benefits for a week or more as a result of failure to report.


9. Number in Item 6 Who Were Neither Rescheduled nor Disqualified. Enter the number of claimants reported in item 6 who were neither rescheduled nor disqualified nor ineligible for a week or more. This includes claimants who stopped claiming EUC08 as well as claimants excused from participation in RES/REA.


10. Number of Claimants Who Failed to Participate in a Reemployment Service. Enter the number of claimants who failed to participate in a reemployment service to which they were referred. This does not include failure to report to the initial REA session, as reported in item 6.


11 Number in Item 10 Who Were Disqualified. Enter the number of claimants reported in item 10 who were disqualified or were ineligible for benefits for a week or more as a result of not participating in a service to which they were referred.
