Abstract: In order to participate in the Medicare program as a CORF, providers must meet federal conditions of participation. The certification form is needed to determine if providers meet at least preliminary requirements. The survey form is used to record provider compliance with the individual conditions and report findings to CMS.

Short Statement: In CMS-359, the time estimate in the form's PRA Disclosure Statement has been corrected to read 15 minutes. The burden has been adjusted to account for respondents who have opted to terminate their participation in the Medicare program (e.g., 476 in 2009 v. 295 in 2012). Also, the information collection requirements contained in 42 CFR 485.54 through 485.66 that are subject to OMB review are currently covered under OMB 0938-1091 (CMS-R-55).
