Terms of Clearance

OMB placed the following Terms of Clearance on this collection when they approved it in 2010:

SSA should report this collection as a violation is OMB's annual ICB report.

To comply with these Terms, SSA reported this collection as a violation in the 2011 ICB report.

Revisions to the Collection Instrument

• In 2010, we reported this as an existing collection in use without OMB approval. At that time, we inadvertently submitted this ICR with an incorrect SSA form number. Since we already use form number SSA-112 on another SSA form, we are correcting the form number here to avoid confusion in members of the public who use this form. The correct SSA form number is SSA-132.

• We are also making the following minor revisions to the form to improve clarity:

• Under page 2 of the heading “Instructions for Completing,” add the words “Notification of a Social Security Number (SSN) to an Employer for Wage Reporting Purposes Form”

• Add the following sentence to the Privacy Act Statement: “Completion of this form is voluntary.”

• We are revising the PRA Statement to reflect our current boilerplate language. The current language, which dates back to the last reprint of the form, is now outdated.

• SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statement on agency forms. As a result, SSA is adding a Privacy Act Statement to this form.
