Who must comply?	Owners and operators (aka applicants) of certain waterfront facilities and vessels.
What is this collection about?	This information requires an applicant to submit an application to the Coast Guard (CG) prior to welding or hot work. It is needed to helps ensure that waterfront facilities and vessels are in compliance with safety standards. Also, the posting of warning signs is required on certain facilities.

When must information be submitted to the Coast Guard?	Information must be submitted to the CG prior to welding or hot work.
How is the information submitted?	In writing or electronically via e-mail. The information is submitted to the CG Captain of the Port (COTP) using form CG-4201. The form is available at— http://www.uscg.mil/forms/cg/CG_4201.pdf. Contact info for CG COTPs can be found at http://www.uscg.mil/top/units/.
What happens when complete information is received?	The CG reviews the information, and if it conforms with the regulations, issues a permit to the applicant.
