At the time of initial approval for this information collection, the DOL agreed to amend the incentives associated with the collection to limit payment to $40 for respondents who call in and $30 to those who wait to be called. The Office of Management and Budget issued clearance terms to allow the changes to be submitted via a nonsubstantive change request. This ICR submits updated documents to address those clearance terms.
