limitations of the data and ensure that evaluation results are

knowledge and behavior or the impact of the campaign on long-term

In 2012, CDC's Office on Smoking and
Health (OSH) conducted baseline and follow-up surveys to assess
awareness of and exposure to tobacco education advertisements that
were launched in the Tips from Former Smokers campaign (TIPS Phase
1). New ads will be fielded in 2013 (TIPS Phase 2). CDC requests
OMB approval conduct an additional follow-up survey in
2013.

There will be an increase in
the number of respondents (from 34,660 to 43,737) for the Phase 2
Screening and Consent Process; an increase in the number of
respondents (from 5,000 to 14,250) for the Smoker Phase 2 Follow-Up
Questionnaire; and an increase in the number of respondents (from
2,000 to 3,286) for the Non-Smoker Phase 2 Follow-Up Questionnaire.
However, because we will discontinue use of the Smoker Baseline
Questionnaire and the Non-Smoker Baseline Questionnaire (which were
fielded during the initial Phase 1 clearance period), there will be
a net reduction of 1,250 burden hours for the Phase 2 Revision
request.
