Under Title 7 of the U.S. Code and CIPSEA (Public Law 107-347), facts about your operation are kept confidential and used only for statistical purposes in combination with similar reports from other producers. Response is voluntary. According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a valid OMB control number. The valid OMB control number for this information collection is 0535-0039. The time required to complete this information collection is estimated to average 15 minutes per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information. |

INSTRUCTIONS FOR REPORTING
1. QUANTITY RECEIVED: Report all quantities received on a pay-weight basis. “Pay-weight” refers to the quantity of fruit delivered and paid for at the rate or rates specified in contracts or purchased agreements. Culls and rejects not paid for, or considered to be of no value, should not be included in the quantity received. Exclude purchases from other processors.
2. AVERAGE PRICE PAID ON A PAY-WEIGHT BASIS: Report average prices paid to growers for the quantities reported in 1.
3. POINT AT WHICH PRICE IS TO BE REPORTED: Report price given on the island of production at the processing plant door or first delivery point. Include in the prices, the delivery cost to the processing plant if fruit is not delivered by grower. Include payment to hired truckers or the cost of company trucks when used. Exclude freight cost from the reported price.
