Abstract: SF 2802 is used to support the payment of monies from the Retirement Fund. It identifies that applicant for refund. SF 2802A is used to comply with the legal requirement that any spouse of former spouse of the applicant has been notified that the former employee is applying for a refund. Standard Form 2802 was revised to enable OPM to comply with Public Law 104-134. The law requires most Federal payments are paid by direct deposit through Electronic Funds Transfer (EFT) into a savings or checking account at a financial institution.
