Subject: OSAP Survey of Exposure Control Plans for Bloodborne Pathogens
Dear Dental Practitioner:
The Organization for Safety, Asepsis and Prevention (OSAP), in partnership with the National Institute for Occupational Safety and Health (NIOSH), Centers for Disease Control and Prevention (CDC), is asking for your participation in an online survey. The survey gathers information on dental practices concerning use of exposure control plans for reducing the risk of exposure to blood and other potentially infectious materials in private dental practices. Invitees have been selected from a distribution list of recipients of dental publications from Aegis Communications.
The survey is anonymous and voluntary. No personal identifiers such as name, address, email address or phone number will be recorded. Your responses to all questions will be kept secure
This survey should be completed by only one person in the primary dental practice, preferably the one who has responsibility for implementing OSHA regulations. That person can acquire input, however, from other staff in the practice. The survey will take about 15 minutes to complete.
At the conclusion of the survey, a report containing aggregate data from all respondents will be published and made available to the dental health community. The data from this survey may lead to improved health and safety of dental care workers.
Please accept our thanks in advance for making a difference and completing this important survey. Your participation is critical to the success of the survey.
Survey Instructions
Responses will be saved as questions are answered, but the respondent cannot exit the survey and then come back to it at a later time. Once the Submit button at the end of the survey is pressed, no changes can be made.
