Subject: Final opportunity to complete the online survey on Use of Exposure Control Plans for Bloodborne Pathogens in Private Dental Practices. If you have not already completed the survey, please do so by midnight on (insert date).


If you have already completed this survey thank you and please disregard this email. Because the survey is anonymous no one will know who completes the survey so we are unable to exclude anyone from follow-up reminders.


Dear Dental Practitioner:


Nearly four weeks ago, we sent you an email asking you to participate in an online survey on concerning use of exposure control plans for bloodborne pathogens in private dental practices. The Organization for Safety, Asepsis and Prevention (OSAP), is partnering with the National Institute for Occupational Safety and Health (NIOSH), Centers for Disease Control and Prevention (CDC) for this survey.


This survey should be completed by only one person in the primary dental practice, preferably the one who has responsibility for implementing OSHA regulations. That person can acquire input, however, from other staff in the practice. The survey will take about 15 minutes to complete.


Your responses to all questions will be kept secure. No personal identifiers such as name, address, email address or phone number will be recorded.


At the conclusion of the survey, a report containing aggregate data from all respondents will be published and made available to the dental health community. The data from this survey may lead to improved health and safety of dental care workers.
