This request is for approval of the
reporting requirements that are contained in the Federal Perkins
Loan Program regulations. Under the regulations, when Perkins
borrowers contact the participating institution regarding a Total
and Permanent Disability discharge, the institution would be
required to refer the borrower to the Department rather than
providing the forms and additional information to the borrower,
which will decrease burden. The purpose of this submission is to
also renew this collection for the next three year period. The
information collection requirements are necessary to determine
eligibility to receive program benefits and to prevent fraud and
abuse of program funds.

additional information from both borrowers and institutions for
tracking purposes and to ensure program benefits are available to
