Kansas City Commodity Office, Warehouse License
and Examination Division, Licensing Branch, uses this form to amend
an existing bond warehouse operators have provided under the United
States Warehouse Act.
The bond is prepared by the Licensing Branch and
sent to the warehouse operator for signature and forwarding to the
warehouse operator’s insurance company for execution. Surety
submits the original of the completed form in hard copy or facsimile
to the Kansas City Commodity Office (KCCO), Warehouse License
and Examination Division, STOP 9148, P.O. Box 419205, Kansas City, MO
64141-6205; or FAX 816-926-1774, for acceptance and licensing.
Warehouse operators execute the document by
signatures and then forward the form to their Surety for execution.
