Thank you for agreeing to participate in our assessment of private well owner outreach and education programs. This effort is being conducted by the Illinois State Water Survey and Illinois Water Resources Center at the University of Illinois. We are working under contract with the Centers for Disease Control and Prevention (CDC).
This assessment seeks to better understand what makes private well owner education initiatives effective by collecting information from departments like yours. We will statistically assess this data and identify a suite of community engagement best practices that you and other environmental health staff at state, local, and tribal environmental health and agricultural departments who are government employees and who conduct outreach to private well owners can use in future efforts. A report will be compiled late in 2014 and you will be e-mailed a courtesy copy.

If you need to close the online system before finishing, the questions will remain open at this unique URL until it is submitted (button after the last question), so you can go back to the URL and finish at a later time. Please answer all of the questions if possible and to the best of your ability.

The data collection consists of 25 questions, both multiple choice and open-ended, and will take approximately 18 minutes to complete. If you have a recurring well owner outreach program, please describe only the last occurrence.
Please note that you may quit in the middle and come back to it, but once it is submitted you will not be able to change your responses. Be sure to read any question-specific instructions if you are unclear. Also, please note question #22, it asks if we can include your responses individually in the appendix of the report. We believe that the individual information will be valuable to others who are interested in developing an outreach program or increasing the success of their existing well owner outreach program.
