Subject: CDC needs your input on state and local Ebola Domestic Inquiries

As you may know, the 2014 Ebola Viral Disease (Ebola) outbreak represents one of the largest Ebola outbreaks in history, and the first in West Africa. The first U.S. clinical inquiry received by the Ebola Response Team at CDC was on July 28, 2014. It is important that the U.S. is adequately prepared to identify and manage potential patients, in order to prevent the spread of disease, and protect healthcare personnel and the American public. To this end, the CDC invites you to complete an assessment of your state, territorial or local health department’s domestic inquiries and preparedness related to Ebola.

Please go to LINK and follow the instructions on the screen to complete the assessment online. The assessment will be open from DATE to DATE and should take about 12 minutes or less to complete.

Your input is very important to us and will help CDC to ascertain the actual number of domestic inquiries state, local, and territorial public health departments have received since the start of the Ebola epidemic, as well as departmental preparedness to respond and guide the development of Ebola prevention protocols. Your feedback will assist us to continue education efforts, and provide guidance to state and local health departments that will assist them in their Ebola response efforts.
