Cryptosporidiosis: diagnostic laboratory and reporting practices among reporting jurisdictions in the United States

Attachment E

Day 7 email to state / jurisdiction health departments

Dear _______________,

I hope this email finds you well! I am an EIS Officer with CDC’s Waterborne Disease Prevention Branch. We are working to understand factors contributing to the ongoing, persistent increase in cryptosporidiosis incidence rates in the United States. We are reaching out to you, the Waterborne Disease Coordinators in the 50 states, Washington, DC, and New York City, to help us gather information on cryptosporidiosis diagnostic laboratory and reporting practices. The findings will help us identify state and local needs and direct development of recommendations and resources for you, our state and local partners.

Last week I sent you an email to request your assistance with this project. Our records indicate that we have not yet received a response from your state and your participation is very valuable to us.

Below you will find a link to the information-collection instrument. It should take an average of 78 minutes to complete. Please complete this survey by DATE.

Your participation is voluntary, and you may refuse to answer any question at any time. All responses will be kept secure and IP addresses will not be collected. Identifiers will not be included in any published materials related to this study. We will summarize the responses from all contacted health departments and share the results in a final report.

Thank you so much for your assistance with this project! Please contact me with any questions you may have.

Best,

Julia
