Changes to SD Form 816/816c

On the SD Form 816 page 1, we changed:

• The School Year (top of the page)

• The Fiscal Year (top of the page)

• The address of where to send the document when completed (top of the page)

• Definition of 'Severely Disabled' (section 1, as advised by DoDEA OGC)

• Eligibility Criteria (section 1, as advised by DoDEA OGC)

• Question #4, added "A minimum of two students must be claimed for eligibility purposes".

On page 2, we changed:

• The School Year (top of the page)

• The Fiscal Year (top of the page)

• Question #3, added "(Required for award processing)".

On page 3, we changed:

• The School Year (top of the page)

• The Fiscal Year (top of the page)

• Added a section requesting the LEA DUNS number, which required renumbering all of the questions on this page.

• At the bottom, we changed the address of where to send the document when completed.

Page 4 is a duplicate of page 2.

The SD Form 816c is an exact duplicate of pages 2 and 4 (from the SD Form 816) with 1 exception.

We added "(Continuation Sheet)" after the title of the document.
