Survey

Job title (of person filling out questionnaire):

When did you begin working at the Colorado State Department of Human Services (year)? ____

1. On a scale of zero to four (0 = not integrated at all, 4 = highly integrated), how integrated do you consider the state’s Colorado Works and Child Welfare services during the following time periods? Consider the state as a whole.

We define integration as the coordination of Colorado Works and Child Welfare policies and processes that link services across service boundaries for newly identified clients, dual-system clients, or clients with multiple co-occurring needs. Integration can be used to describe a range of activities, such as several agencies providing a service at one location and the creation of shared planning and budgeting. We use the term integration here to describe the activities taking place between welfare and child welfare agencies.

Select one number for each year. You may indicate “Don’t know” if you were not working with the State’s Department of Human Services at the time, or if you do not know.
