INSTRUCTIONS AND RESPONSIBILITIES

HEALTH CERTIFICATE: Shipment MUST be accompanied by required Health Certificate. A VALID, VS FORM 17-135A, Permit to Transit, must accompany the shipment. THE SHIPPER must make two copies of the permit and complete items 1, 2, 3, 5, and 6. Designate one copy as “Carrier Copy” and one copy as “Custom Copy.” THE CARRIER copy of this permit must accompany each shipment into the United States. The carrier copy is to be furnished to initial carrier. THE SHIPPER shall make certain that the Health Certificate which accompanies the shipment has been issued (signed) or endorsed by a salaried veterinary officer of the National Government of country of origin and deliver the “Carrier’s Copy” of the permit to the initial transporting carrier. THE INITIAL CARRIER shall not accept the shipment unless the accompanying Health Certificate has been (signed) or endorsed as outlined in the above paragraph; will make sure that the Health Certificate and the permit accompanies the shipment to the U.S. port of entry, and arrange for the “Carrier’s Copy” to be available to the final carrier who will transport the shipment to the U.S port of entry. THE IMPORTER sends the “Custom Copy” to the Port Veterinarian before shipment is scheduled to arrive. U.S. INSPECTOR AT PORT OF ENTRY will hold the “Custom Copy” until the shipment is received, then forward to the U.S. Collector of Customs. The “Carrier Copy” of the permit will be retained by the VS Veterinarian at U.S. port of entry. |
