Please enter the serial/registration number(s) of the application(s) for which you wish to
change the owner's address for an application or registration (which may be the same as, or
differ from, the correspondence address). If the requested change is for an existing
registration, you must still enter the original serial number assigned to that filing, accessed by
entering the registration number in TARR, if necessary. To change the owner's address for
more than one application or registration having the identical owner, enter multiple serial
numbers (but not exceeding 300 serial numbers per request form, separating each with a space,
with no punctuation). Click on the "Continue" button below to access the form for entering
your information.
Changing the owner's address, whether through this electronic form or filing a paper change
request, does not record or effect changes to the applicant's or registrant's name, nor record or
effect transfers of ownership of an application or registration from one party to another. You
must file such recordals with the Assignments Division. For more information on

Changing the owner's address, whether through this electronic form or filing a paper change
request, does not effect changes to the correspondence address that the Office will use
UNLESS the owner's address and the correspondence address are identical. To change the
correspondence address for an application or registration, please use the Change of
Correspondence Address form.
Note to Registrants: Use of this form to change the owner's address will not automatically
generate an "updated registration certificate" (URC). To obtain an URC that reflects the new
address, you must submit a written request for the URC pursuant to §7 of the Act, along with
the required fee. See 37 C.F.R. §2.173(e).
Note:You must complete any field preceded by the symbol "*". ONLY the applicant or its
attorney is qualified to sign this form requesting a change of the owner's address data.

Note: Do NOT use this form for applications or registrations
based on Section 66(a) of the Act. You must file a change
to the name or address of a holder of an extension of
protection of an international registration to the United
States directly with the International Bureau (IB) of the
World Intellectual Property Organization, through Form
MM9. Madrid Protocol Article 9bis. Once the change is
recorded in the International Register, the IB will notify the
USPTO and the USPTO will update its records.

The information collected on this form allows the PTO to determine whether a mark may be registered on the Principal or
Supplemental register, and provides notice of an applicant's claim of ownership of the mark. Responses to the request for
information are required to obtain the benefit of a registration on the Principal register. 15 U.S.C. §1051 et seq. and 37 C.F.
R. Part 2. All information collected will be made public. Gathering and providing the information will require an estimated 15
or 21 minutes (depending if the application is based on an intent to use the mark in commerce, use of the mark in
commerce, or a foreign application or registration). Please direct comments on the time needed to complete this form, and/
or suggestions for reducing this burden to the Chief Information Officer, U.S. Patent and Trademark Office, U.S.
